    ..your password is printed on the hardcopy of the form that came with the test samples. Please do not forget to mail or deliver the completed RESULTS FORM to the National Reference Laboratory, East Avenue Medical Center, Diliman, Quezon City.
  • PT Cycle Results
    ..this will be posted 8-12 weeks after the distribution of test samples.  The participating laboratories shall also be informed by mail of the results of the PT.
